Key Responsibilities:

This list is intended to summarise the key responsibilities and is not intended to cover
every task that may be required of the role: –

  • Following induction and a period of supervised practice you will work independently with residents within Fonthill delivering care and rehabilitation according to plans and goals set by the nursing team.
  • To work with the multi-disciplinary team in monitoring therapeutic interventions.
  • To maintain knowledge, skills and attitudes necessary to deliver a quality service that is responsive to the changing needs of residents.
  • To provide support to the team with the allocation of daily visits.
  • To provide support to the team with maintaining accurate caseload on the data system.
  • To maintain accurate and contemporaneous electronic records, supported by use of mobile technology I.e. IPAD and iPhone.
  • To deliver care in accordance with Trust Policies, procedures and guidelines, according to competencies.
  • To report any changes in condition of the resident to the Intermediate care practitioner/operational lead, following standard operating procedure for escalation and handover.
  • To respect confidential information obtained in the course of work and refrain from disclosing such information to anyone other than professional staff directly concerned with the residents care or other authorised person.
  • To be aware of the Data Protection Act, and follow local codes of practice to ensure appropriate action is taken to safeguard confidential information.
  • Assist the therapist with installation and monitoring effective use of equipment.
  • Assist the team to monitor an individual’s progress towards set goals.
  • Assist the RN in the technical aspects of nursing care e.g. Dressings, glucose monitoring etc. following a period of training and when deemed competent to do so.
  • To carry out administrative and clerical duties as delegated.
  • To have the ability to work unsupervised, planning and organising tasks.
  • To take part in surveys and audits as required.
  • To participate in annual appraisal and personal development plans.
  • To participate in training as appropriate to personal and team skills development.
  • To participate in clinical supervision.
  • To promote the safety and wellbeing of patients, staff and other persons at all times, and assist in ensuring a safe working environment.
  • To be responsible for the ordering of stock, checking of equipment and any other reasonable duties.
  • To ensure that all complaints, untoward incidents, accidents and areas of clinical risk are identified and reported in accordance with Trust policies and procedures.
  • The post holder must meet the daily workload requirements.
  • To maintain knowledge and comply with the Trust’s corporate and local policies and procedures and in particular the Trust’s policies on equal opportunities.
  • To undertake such other duties as may be required from time to time and are consistent with the responsibilities of the role.

£11.00 Per Hour
