We are now looking for outstanding people to join our team at Fonthill House.

As a Learning Disability Nurse, you will show warmth and support to our residents, helping to promote independence and dignity and positively enhancing the quality of lives of the individuals in your care. You will predominantly be providing direct care to people.

Your duties and responsibilities will vary depending on the needs of the person but can include:

Being aware of safeguarding protocols and policies.
Ensuring the highest possible level of care is maintained by supporting/assisting and assessing people, when required, with washing, dressing and all other aspects of daily living.
Working according to the NMC Code of Professional Conduct and other professional and legislative guidelines as a named nurse for a number of residents, taking responsibility for:
- Protecting, supporting and improving the health of individual residents.
- Assessment of holistic care needs for residents.
- Development, implementation and evaluation of care and treatment in collaboration with the individual resident.
Undertaking active participation in effective discharge planning.
Actively undertaking management and leadership of staff in the absence of more senior nursing staff.
Supporting the manager/senior nurses in all aspects ensuring high standards of care delivery and compliance.
Ensuring optimum quality of care for residents at all times.
Proactively undertaking developmental work to assist the nursing team in all aspects of continually improving the standards of care delivery; the measurable compliance of the service and increasing the knowledge base of the service in the treatment and recovery of residents.
Working proactively and ensuring others within the nursing team also work proactively within the wider multidisciplinary team, fostering teamwork and ensuring the holistic assessment and treatment needs of the residents are being met.
Taking an active role in the training and education of nursing staff to achieve high-quality care through innovative and evidence-based practice.
Acting as an ambassador for the service and organisation through all interactions upholding the reputation and professionalism of the team.

Main Duties & Responsibilities

Acting as a named nurse for identified resident(s), ensuring all aspects of care planning are implemented and reviewed according to relevant protocols.
Supporting more junior staff in undertaking named nurse responsibilities.
Being professionally responsible and accountable for all aspects of your work including assessment, intervention, evaluation of outcomes, equipment use, resources and documentation.
Ensuring that residents are actively engaged and involved in their care and treatment.
Liaising with all relevant staff to ensure that the 24-hour multidisciplinary service is delivered effectively.
Ensuring and continually improving the quality of individual evidence-based care planning.
Being professionally responsible and accountable for all aspects of your work including assessment, intervention, evaluation of outcomes, equipment use, resources and documentation.
Ensuring all residents have the opportunity to participate in risk assessments. Making sure they are relevant, clearly identify risks and any risk management plans are clear and consistent.
Ensuring policies for the safe storage, custody and safe administration of medicines are carried out to the highest standard and undertaking activities to improve systems.
Assisting the management and nursing team in ensuring that all clinical documentation/procedures comply with all applicable legislation.
Taking positive action locally where concerns around legislative compliance occur and ensuring effective reporting to managers of any concerns.
Undertaking to provide clear concise, accurate and objective documentation in the form of:
- Reports
- Daily recording entries
- Measurement and result recording
- Medicine administration records
- Security forms
- Other documentation related to job role
Ensuring correct procedures take place for the admission and discharge of residents.
Pro-actively encouraging, directing, supporting and training staff nurses in the management of workflows.
Maximising the use of resources and reducing waste/usage where possible.
Maintaining a professional position at all times and ensuring others around you do also.
Maintaining a personal professional portfolio and ensuring that you implement evidence-based clinical practice.
Undertaking and promoting active engagement in clinical supervision, carrying out clinical supervision for staff nurses as required.
Taking an active stance in the professional development and support of staff nurses and others influenced by the role.

Health & Safety

Attending all health and safety updates and training required as part of mandatory training.
Assisting in the maintenance of physical security and safety through regular monitoring and checks.
Actively undertaking responsibility for the health, safety and welfare of residents and other employees in accordance with legislation and specific policies and procedures.
Carrying out any other tasks that may be reasonably assigned to you.
